Scholarship Application Request

The Application Request Form provides initial vital information to determine applicant eligibility and tracks the date and time of each request.

• All scholarship applicants must complete the Request Form, not a parent, friend or another person. A request submitted by anyone other than the applicant will be disallowed. Returning applicants may complete the application itself by selecting the option below.
• Following submission, the new applicants will receive a confirmation by email confirming receipt.
• Upon confirmation, the applicant will receive notification by email confirming eligibility. Applicants whose eligibility cannot be confirmed will also be notified, or be informed that additional information is required.
• Eligible applicants will be provided access to the Scholarship Application and Instructions.
